En la literatura internacional se crea la idea de que la información científica sobre el síndrome de intestino irritable (SII) en México y Latinoamérica es escasa. Sin embargo, se desconoce el número y la tendencia de las publicaciones sobre SII en la región.
ObjetivoDeterminar el número y tipo de publicaciones sobre SII en México y Latinoamérica entre 1990-2015.
MétodosBúsqueda sistemática en Medline empleando los términos «síndrome de intestino irritable» o su abreviatura (IBS) en inglés, cruzándolo con México y cada país de Latinoamérica. Los artículos se seleccionaron según hubieran sido publicados en México y/o Latinoamérica; publicaciones internacionales sobre SII en la región o por autores mexicanos y/o latinoamericanos. Además se clasificaron en categorías: Revisión/Editorial/Cartas al Editor (REV/ED), Epidemiología/Calidad de vida/Factores psicosociales (EPI/QOL), Diagnóstico (DX), Tratamiento (TX), Traslacional/Fisiopatología (TRAS).
ResultadosSe identificaron 66 publicaciones de México y 47 de Latinoamérica pero solo un 87.7% y un 48.9% respectivamente se ajustaron a los criterios de selección. La distribución de categorías fue: EPI/QOL, 36.2%; REV/ED, 27.5%; TRAS, 18.8; TX, 10%; DX, 7.5%; similar entre México y Latinoamérica (p=0.33). Las publicaciones se incrementaron a partir del 2005 con un pico en 2010 en México, y se mantuvieron estables a lo largo de los años con un pico en 2012 en Latinoamérica.
ConclusiónEn la última década se observa un aumento notorio en la investigación sobre SII en México y Latinoamérica. Si bien predominan los estudios EPI/QOL, la sofisticación se refleja en los TRAS que ocupan el tercer lugar. Lo anterior demuestra una madurez que permite realizar consensos basados en datos locales.
There is an implied perception that little scientific information on irritable bowel syndrome (IBS) comes out of Mexico and Latin America in the international medical literature, but the number and tendencies of articles from the region on IBS are not known.
AimsTo determine the number and type of Mexican and Latin American articles on IBS published between 1990-2015.
MethodsA systematic search of Medline was carried out employing the terms «Irritable Bowel Syndrome» or its abbreviation (IBS). Mexico and every other Latin American country were added to specify the search. The articles were selected if they were published in Mexico and/or the rest of Latin America, if they were international papers on IBS in the region, or if they were written by Mexican and/or Latin American authors. In addition, the articles were classified into the following categories: Reviews/Editorials/Letters to the Editor (REV/ED), Epidemiology/Quality of Life/Psychosocial Factors (EPI/QOL), Diagnostic (DX), Treatment (TX), and Translational/Pathophysiologic (TRANS) studies.
ResultsA total of 66 articles from Mexico and 47 from Latin America were identified, but only 87.7 and 48.9%, respectively, fit the selection criteria. Category distribution was: EPI/QOL: 36.2%, REV/ED: 27.5%, TRANS: 18.8, TX: 10%, and DX: 7.5% and was similar between Mexico and Latin America (p=0.33). The year 2005 saw an increase in the number of articles in Mexico, with a peak in 2010. The figures for Latin American articles remained stable over the years, with a peak in 2012.
ConclusionsA noticeable increase has been observed over the last decade in the research conducted on IBS in Mexico and Latin America. Even though the EPI/QOL studies predominate, the sophistication of research is reflected in the TRANS studies that are in third place of frequency. The information referred to herein demonstrates maturity in the field, making it possible to carry out consensuses based on local data.
Existe la percepción tácita de una baja producción científica en el campo de los trastornos funcionales gastrointestinales en especial del síndrome de intestino irritable (SII), en Latinoamérica. De hecho, una revisión sistemática de la literatura, sobre la prevalencia global del SII basada en estudios en población abierta, concluyó que faltan estudios sobre la prevalencia y características epidemiológicas de dicho trastorno, especialmente de Centroamérica y son escasos los de Suramérica1. Solo identificaron 4 publicaciones de Suramérica, de las cuales una no fue realmente en población abierta2. Lo anterior es importante ya que el SII tiene una incidencia reportada del 9-16%3 y se encuentra entre los 5 trastornos gastrointestinales más frecuentes4.
Debido a que nuestra percepción es que existe un incremento en la investigación sobre el SII en la región, nuestro objetivo fue realizar una búsqueda en Medline de los últimos 25 años. Nuestra hipótesis fue que ha habido una explosión en las publicaciones sobre SII en México y Latinoamérica.
MétodosBúsqueda en Medline del 1 de enero de 1990 hasta el 1 de abril de 2015. Debido a nuestro interés específico, se realizó la búsqueda sobre México separadamente del resto de Latinoamérica. Se utilizaron los términos en inglés: «Irritable Bowel Syndrome» o «IBS», cruzándolos con «Mexico» o «Latin America». Debido a que con «Latin America» no se encontró ningún resultado, se procedió a repetir la búsqueda con cada uno de los países de Latinoamérica. Dos autores (RP, MS) revisaron de manera individual los resúmenes de cada uno de los artículos identificados y, en caso de ser necesario, los artículos completos, para corroborar que cumplieran los criterios de selección. Estos fueron: 1. Artículos sobre SII publicados en México y/o Latinoamérica; y/o 2. Artículos internacionales que se refirieran al SII en la región; y/o 3. Artículos sobre SII por autores mexicanos y/o latinoamericanos. Posteriormente, los mismos 2 autores se reunieron para consensuar cuáles publicaciones sí correspondían a dichos criterios de selección y excluyeron aquellas que en realidad no fuesen sobre SII, o eran sobre SII pero no de México o Latinoamérica o que no fueran de autores con afiliación en la región. Además, los artículos fueron clasificados en 5 categorías: Revisión/Editorial/Cartas al Editor (REV/ED), Epidemiología/Calidad de vida/Factores psicosociales (EPI/QOL), Diagnóstico (DX), Tratamiento (TX), Traslacional/Fisiopatología (TRAS). En los resultados se describen los artículos por año, país de los autores y las categorías correspondientes. La frecuencia de las categorías se comparó entre México y Latinoamérica. Se describe el número de artículos promedio por año, y para determinar si habían aumentado las publicaciones en el periodo de tiempo de interés, se graficó el número de artículos por año, separando las de México de las del resto de Latinoamérica. Los resultados se describen en frecuencias y las comparaciones se realizaron mediante la prueba exacta de Fisher. Se consideró significativa una p<0.05.
ResultadosSe identificaron 66 publicaciones de México y 47 del resto de Latinoamérica, de las cuales 57 (87.7%) y 23 (48.9%) respectivamente, cumplieron los criterios de selección. En la tabla 1 se muestran los artículos de acuerdo a las diferentes categorías, siendo en orden de frecuencia los EPI/QOL: 36.2%, REV/ED: 27.5%, TRAS: 18.8, TX: 10% y DX: 7.5%. No hubo diferencias en las categorías entre México y Latinoamérica, EPI/QOL: 42.1 vs. 21.3%, REV/ED: 24.5 vs. 34.8%, TRAS: 19.3 vs. 17.4%, TX: 7 vs. 17.4%, DX: 7 vs. 8.7%; p=0.33.

En México se publicaron aproximadamente 3 artículos/año sobre SII a partir de 1997, mientras que en el resto de Latinoamérica fueron 1.5/año a partir de 2001. Adicionalmente, en México se presentó un incremento a partir del 2005 del 84.2% con un pico en el 2010. (fig. 1). En el resto de Latinoamérica si bien la mayoría de las publicaciones (82.6%) se presentan a partir del 2005, se mantuvieron estables a lo largo de los años con un pico a partir del año 2011 pero no se encontró ninguna referencia del 2014 (fig. 1).
DiscusiónEn la presente revisión en Medline hemos identificado un incremento en las publicaciones sobre SII en México y el resto de Latinoamérica principalmente a partir de la segunda mitad de la década del 2000. Sin embargo, hay que anotar que en México se publica más del doble que en el resto de los países de la región. Los artículos más frecuentes son aquellos relacionados con aspectos epidemiológicos, calidad de vida y aspectos psicosociales.
Si bien hemos identificado un número importante de publicaciones en Medline, hay que anotar que existe un sesgo de publicación en este repositorio, a tal grado que artículos publicados en revistas locales con estudios epidemiológicos bien realizados no se encuentran citados. Tal es el caso de un estudio sobre la prevalencia del SII de Uruguay5 y otro de Venezuela6. Por otra parte, también observamos que en la búsqueda no se identificaron algunas cartas al editor7 e incluso editoriales recientes sobre el SII8 publicadas en revistas que sí se encuentran referenciadas en Medline. Lo anterior indica que para realizar una revisión adecuada sobre el SII en México y Latinoamérica se requiere también analizar otros repositorios como BIREME/Lilacs o SciELO.
Con respecto a las categoría de las publicaciones, no fue sorprendente encontrar que la mayor parte correspondió a artículos epidemiológicos, calidad de vida y aspectos psicosociales, quizá porque son los más sencillos de realizar y en general solo requieren la aplicación de cuestionarios. Por otra parte, el agrupar esos 3 aspectos en una sola categoría puede explicar el mayor número de artículos en la misma. Para nuestra sorpresa, los artículos traslacionales y sobre fisiopatología que requieren técnicas de laboratorio e investigadores especializados –que se hubiese pensado que serían los menos frecuentes– ocuparon el tercer lugar en frecuencia, lo cual indica la sofisticación cada vez mayor de la investigación en el campo en México y Latinoamérica. Finalmente, vale la pena mencionar los artículos de revisión que en algunos casos incluyen colaboraciones entre autores de Latinoamérica y otras regiones del mundo9,10. Lo anterior sugiere la importancia y liderazgo cada vez más creciente a nivel internacional de algunos investigadores latinoamericanos en el campo del SII.
Por último, hay que anotar la mayor producción científica sobre SII en México que en todo el resto de países Latinoamericanos. Es muy probable que esto se deba a la presencia de mayores recursos para la investigación en este país, y sobre todo, el cada vez más reconocido y remunerado trabajo de aquellos médicos y científicos básicos en academia. Dichos recursos parecen seguir siendo escasos en el resto de los países de Latinoamérica, y aquellos que hacen investigación y publican en esos países realizan un esfuerzo que debe ser reconocido.
En conclusión, en los últimos años se observa un aumento importante en las publicaciones sobre SII en México y Latinoamérica, lo cual muestra la madurez cada vez mayor en este campo en la región y nos permite ya tener consensos basados en nuestra propia información. Por otra parte, al realizar revisiones sistemáticas, se requiere también la búsqueda en repositorios locales para que esta sea adecuada y no sesgada.
